The goal of case management/care coordination is to help youth and families remain together with substantial community based services to meet their on-going needs. Mental Health Case Management services assist individuals by linking them to rehabilitation and treatment services, entitlements/benefits for which they may be eligible, housing, social supports, vocational and educational services. Medicare and private insurances do not pay for case management services. Individuals who are eligible for case management services cannot be receiving case management services from another agency and must be willing to voluntarily agree to participate in case management services.
